Holy Family Medical Center in Des Plaines, Illinois is currently seeking an experienced and compassionate Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our Primary Care team . This full-time opportunity offers a chance to deliver comprehensive outpatient care in a collaborative, patient-centered environment. Position Details

• Full-time position in an outpatient primary care clinic
• Polish language skills preferred to meet the needs of the local community
• Open to Nurse Practitioners or Physician Assistants
• See patients for preventive care, chronic disease management, and acute visits
• Collaborate closely with a team of physicians, APPs, and clinical support staff
• Manage your own patient panel with opportunities for continuity of care
• 40-hour/week schedule with minimal call responsibility
• Average of 16-24 patients per day
• Supportive, mission-driven care environment

Qualifications

• Licensed NP or PA in the state of Illinois
• Board Certification in Family Medicine (NP) or appropriate PA certification
• Minimum of 2 years of Primary Care or Family Practice experience preferred
• New graduates with strong clinical rotations encouraged to apply

A reasonable compensation estimate for this role, which includes estimated wages, benefits, and other forms of compensation, is $110,000.00 to $135,000.00 on an annualized basis. The exact starting compensation to be offered will be determined at the time of selecting an applicant for hire, in which a wide range of factors will be considered, including but not limited to, skillset, years of applicable experience, education, credentials and licensure.
